I acknowledge that all information provided in this form is accurate to the best of my knowledge and belief. Candidates must be able to provide written proof of training or experience and/or documentation of education completion upon request. Candidates are solely responsible for knowing and understanding any state regulatory requirements that may apply to their certification and/or ability to practice their profession in their particular state.


Your certification, like many credentials, will remain active during the certification period. Certification does not guarantee any candidate’s ability to meet some or all regulatory requirements related to their profession.


Check with your employer if ANY restrictions may apply to you before registering. CG TRAINING CENTER will not be responsible if you fail to do so, and all applicable cancellation policies and fees will apply. Please read our Terms and Conditions before submitting your registration.


Removing or attempting to remove exam questions or other assessment material from the testing site is prohibited. Federal copyright law prohibits copying, reproducing, recording, distributing, or disclosing these assessment questions by any means, in whole or in part. Violations may result in civil and criminal penalties. I certify that I will not cheat or violate the confidentiality of the exam.

Cheating or violating confidentiality may be defined as, but is not limited to, the following:

  • Receiving assistance from anyone during the exam, including peers, friends, or family members.
  • Communicating with other candidates in any form (verbal, written, or electronic).
  • Using unauthorized materials such as notes, books, electronic devices (including non-programmable calculators), smartwatches, or any other device capable of storing information.
  • Reproducing, copying, or distributing exam questions or tests in any way.
  • Memorizing and reproducing questions from previous exams.
  • Requesting or providing information about exam questions to other candidates.
  • Discussing exam content with anyone before, during, or after the exam.
  • Falsifying any exam-related documents.
  • Attempting to influence the score of another candidate or one’s own score.

Academic integrity is fundamental in this process. Any attempt to provide false or misleading information—whether during the application, registration, or exam—will be considered a serious violation of the rules. CG TRAINING CENTER is committed to maintaining the confidentiality of your personal data, except when disclosure is necessary to comply with legal requirements or to ensure the validity of the certification. In cases of non-compliance, sanctions may include exam cancellation, certification revocation, and notification to the appropriate authorities.

Institute Calendar

The Institute will observe the following holidays, and there will be no classes on those days. Holidays are not counted as part of the contracted schedule and are accounted for in your contract.

  • Martin Luther King Jr. Day – third Monday in January
  • Memorial Day – last Monday in May
  • Independence Day – July 4
  • Labor Day – first Monday in September
  • Thanksgiving Day – fourth Thursday in November
  • Christmas Day – December 25

Enrollment Requirements

Students can enroll in courses up to one week (5 business days) before classes begin. Students must bring the following:

  • Valid identification (government-issued ID, driver’s license, or passport)
  • High school diploma or GED certificate
  • Initial payment for administrative and tuition fees (we accept cash and all credit cards)

Institute Policies

Entry Requirements

  • All students must be at least seventeen (17) years old on or before the first day of class.
  • Students must present a valid form of identification (driver’s license or passport).
  • Students must possess a High School Diploma or a General Educational Development (G.E.D.) certificate.

Attendance Requirements

CG Training Center records daily attendance for each student in accordance with state guidelines. Student attendance records will be maintained and available for review by students. The Institute requires students to attend at least 80% of the program. Absences exceeding 20% of the total program are grounds for dismissal. Students with more than 20% absences will have their case reviewed by the Institute Director, with the possibility of being withdrawn from the program. Students who anticipate their absences will exceed 20% of the program have the option to request an official leave of absence.

Leave of Absence

Leaves of absence will be granted to students who apply, subject to the following guidelines:

  • The leave request must be submitted in writing to the attendance officer.
  • The request must include the start date of the leave and the expected date of return to classes.
  • Leaves will be honored within the terms of the student’s Enrollment Agreement and will not extend beyond the contract. If a leave causes the student to extend past the contract end date, the student may be reinstated under a new or modified contract. If the student does not return within the contracted time and does not modify the contract, the contract will be terminated. Where applicable, the student may be entitled to a refund in accordance with the Institute’s refund policy.

Note: Each situation will be handled privately. The Institute will do its best to assist students in achieving their educational goals. A meeting with the attendance officer is required before returning to class. Absences extending beyond the original contract may incur additional tuition costs. Students should be prepared to make up missed classes before rejoining the program.

Make-Up Classes

Hours missed due to absences will be recorded as absences. Students are responsible for making up classwork and assignments. Missed classes must be made up to meet minimum attendance and graduation requirements. Students must meet with the attendance officer before returning to class.

Making Up Work

To achieve their educational goals, students must receive instruction in all areas of the program. Classes missed due to absences or leaves must be made up. Students must complete make-up classes and assignments within five (5) business days of returning to the Institute. Refer to the Institute’s make-up schedule for available days and times. Students should meet with their instructor to receive missed assignments. Students who do not comply with the Institute’s make-up policy may need to arrange private tutoring at an additional cost. Private tutoring costs $20 per hour. If the student opts to wait until the missed lesson is offered in another class, they should be aware that this may change their completion date. The student will need the Institute Director’s approval for any change in the completion date, which may result in a contract modification.

Tardiness

Developing a good work ethic is an important part of training at CG Training Center. Students who arrive late disrupt the instructor and other students. The following system will be used to record tardiness:

  • 1 to 15 minutes late will be recorded as 15 minutes of tardiness.
  • 16 to 30 minutes late will be recorded as 1 hour of tardiness.

Students are responsible for making up missed work. CG Training Center encourages students to plan to arrive at least 10 minutes before class starts.

Code of Conduct

The following behaviors are unacceptable and will not be tolerated:

  • All forms of prejudice, including race, ethnicity, gender, disability, national origin, and creed, demonstrated through verbal and/or written communication and/or physical acts.
  • Sexual harassment, including creating a hostile environment and coercing someone to perform sexual favors in exchange for something.
  • Any proven dishonesty, including cheating, plagiarism, intentionally providing false information to the institution, forgery, and alteration or misuse of institutional identification documents with intent to defraud.
  • Intentional disruption or obstruction of teaching, research, administration, disciplinary procedures, meetings, public programs, or other Institute activities.
  • Physical or mental abuse of any person on Institute premises or at Institute-sponsored or supervised functions.
  • Theft or damage to Institute property or property belonging to a member of the Institute.
  • Failure to comply with instructions from institutional officials acting in the performance of their duties.
  • Violation of laws on Institute premises in a way that affects the Institute’s educational objectives, including, but not limited to, the use of alcoholic beverages and/or controlled dangerous substances on Institute grounds.

Grounds for Dismissal

Students may be dismissed from the Institute for the following reasons:

  • Failure to comply with the Institute’s rules, regulations, policies, and code of conduct.
  • Absence exceeding 20% of instructional time.
  • Failure to maintain the minimum required GPA.
  • Failure to meet financial obligations to the Institute.

The Institute Director will notify the student in writing if dismissal is necessary. The dismissal letter will include the date and reason for dismissal.

Re-Enrollment Policy

Students who have been dismissed from the Institute and wish to re-enroll must submit a written request to the Institute Director.

  • Students dismissed due to excessive absences (more than 20%) or financial reasons may be allowed to re-enroll within the same Institute term.
  • Students dismissed for not maintaining the minimum GPA may receive tutoring and be allowed to re-enroll.
  • Students dismissed for unacceptable conduct may be required to meet with a review panel before re-enrolling. The decision of the review panel is final, and the student will receive a letter within five business days from the Institute Director communicating the panel’s decision.

Student Complaint/Appeal Process

Students who have a complaint or wish to appeal a dismissal must submit a written request for a meeting with the Institute Director. The written request must include:

  • Full name of the student, last four digits of their Social Security Number (or student ID number), and current address.
  • A statement describing the issue, including dates, times, instructors, and if applicable, other students involved.
  • Date of the complaint letter and student’s signature.
  • Three dates when the student is available to meet with the Institute Director and/or appeals panel. These dates must fall within 10 business days of the complaint.

The Institute Director will notify the student in writing of the meeting date where concerns or appeals will be addressed. Every effort will be made to resolve the issue amicably. If necessary, a panel of instructors will hear the concerns and assist in resolving the issues and/or appeals.

The outcome of the meetings will be communicated in writing to the student within five business days. The decision of the Director and/or appeals committee is final. If the contract is canceled by the student or the Institute, the date on the complaint letter will be used to calculate any refund according to the Institute’s refund policy.

Grading System

Competencies taught in each program offered at CG Training Center will be evaluated through written exams and skill development tests. The minimum passing grade to graduate is 75 percent. Students scoring below 75 percent will not receive a Certificate of Completion and may be withdrawn from the program.

Incomplete Grades

Incomplete grades are granted when a student is unable to complete a course due to illness or other serious issues. An incomplete grade may also be assigned if the student fails to submit assignments or attend exams due to negligence or procrastination.

A student who misses a final exam must contact the instructor within twenty-four hours of the exam to schedule a make-up exam. If the student fails to make arrangements to take missed exams, a failing grade will be assigned.

Students who withdraw while passing the program will receive a WP (Withdrew Passing). Students who withdraw without meeting the minimum grade average will receive a WU (Withdrew Unsatisfactory Grades).

Academic Probation for Below-Average Grades

Students who fail to maintain the minimum grade average of 75 percent required for graduation will be placed on academic probation. The student will be scheduled for instructor support according to the recovery calendar.

Students who cannot raise their grade average may be expelled from the program. Refunds will be issued in accordance with the Institute’s refund policy.

Student Assessment Techniques

A test may be administered after each lesson to determine the level of learning achieved. Scores below 75% indicate that the student has not acquired the competencies necessary for employment.

Assessments will include both cognitive and psychomotor tests. Psychomotor evaluations will be compared with industry standards.

This allows students to track their progress. Other assessment methods may include oral exams, skill development tests, practical skills evaluations, and both individual and group projects.

Instructor Assessment Techniques

Instructor evaluations are conducted at the end of each program. The Director reviews the evaluations and uses them to improve the quality of instruction.

Each student may also evaluate their instructor, providing feedback that contributes to teaching improvement.

Withdrawal from the Institute

Students must fill out a withdrawal notice and submit it to the Institute Director. This document must include the student’s name, identification number, and official date of withdrawal.

All financial obligations between the Institute and the student will be calculated based on the official withdrawal date. It is the student’s responsibility to officially withdraw from the Institute.

Failure to do so may result in failing grades, contract violations, dismissal, and additional financial obligations.

Academic Records

The Institute will retain student records until the school’s closure. At that time, records will be forwarded to the New Jersey Department of Labor and Workforce Development.

Upon graduation, students will receive a copy of their records. Students are encouraged to keep these records indefinitely. The Institute maintains the following records:

  • Attendance records
  • Academic progress and grades
  • Financial records
  • Enrollment agreement
  • Record of credits awarded for prior training

Student records are kept by the Institute’s administration office and may be accessed by the student at any time. Students are encouraged to update their records, for example, in the case of address or financial aid changes. All records are private and treated confidentially.

Refund Policy

Students may request a refund of tuition only if the request is made within the first 48 hours after enrolling or paying for the course. After this period, no refund requests will be accepted for any reason, except in the case of course cancellation by the institution.

If a student wishes to cancel their enrollment after the initial 48-hour period, no refund will be issued, regardless of the reason provided.

To formally request a refund, the student must send an email to sales@cgintitutecenter.com including their full name, supporting documentation, and the reason for the request. The institution commits to processing the refund request within a maximum of five (5) business days.

Additional Considerations

Online vs. In-Person Courses / Course Cancellation by the Institution

The institution reserves the right to cancel a course, in whole or in part, at any time due to circumstances beyond its control, such as force majeure, insufficient enrollment, or changes that prevent the course from being conducted.

In case of cancellation, the institution will notify students as early as possible and offer reasonable alternatives, such as a proportional refund or the option to enroll in another course.

Course Modifications

The institution reserves the right to make changes to course content, schedule, methodology, or instructors, provided such changes do not fundamentally alter the nature and objectives of the course.

In case of significant changes, the institution will notify students in advance and offer reasonable options, such as transferring enrollment to another course or receiving a proportional refund.

Unannounced School Closure

If a school closes without notice, enrolled students must contact the Training Evaluation Unit of the Department of Labor and Workforce Development within ninety (90) calendar days of the closure.

Failure to do so within the ninety (90) days may exclude the student from available forms of assistance.

Tuition Refund Policy

If a student’s enrollment ends or the student withdraws for any reason, refunds will be processed according to the following refund schedule:

  • Students wishing to cancel enrollment must do so in writing. The notice must include the withdrawal date and be dated and signed by the student.
  • Refunds will be based on the week the written notice is received. It is recommended to hand-deliver the letter and obtain a signed copy from Admissions, or send it via certified mail.
  • A full refund will be issued if the applicant is not accepted by the Institute or if the student withdraws within three (3) business days after the enrollment agreement is signed by both parties, even if instruction has begun.
  • Cancellation after the third (3rd) business day, but before the first class, will result in a full refund minus administrative fees.
  • For courses of 120 hours or less, the Institute may retain the administrative fee plus a weekly pro-rata portion of tuition.
  • Withdrawal after instruction begins is subject to the following refund policy for programs of 300 or more instructional hours.

An instructional hour is defined as 60 consecutive minutes including attendance, instruction, preparation, and project clean-up.

Refund Schedule

For Withdrawal or Cancellation

During the first week or up to 10% of the program 

20% of tuition + registration fees will be retained 

During the second or third week or up to 25% of the program

50% of tuition + registration fees will be retained 

After the third week or up to 49% of the program

75% of tuition + registration fees will be retained 

After completing more than 50% of the program 

100% of tuition will be retained (no refund) 

A student who has a student loan and withdraws from the program is responsible for notifying the lending institution of the withdrawal from the Institute. This notification must be made in writing. It must include the date of withdrawal, the student’s social security number, and the student’s signature. Students should keep a copy of this letter for their records. CG Training Center encourages the development of good business practices among its students. In this regard, we remind students who are withdrawing that the withdrawal date in the letter to the Director should match the date in the letter to the lending institution.

Tuition and Additional Costs

The administrative fee is non-refundable. Additional costs must be paid on the first day of class. Students who carelessly damage Institute property or equipment may be required to pay for repairs or replacement. Students must fulfill all financial responsibilities before a Certificate of Completion will be issued.

Facilities

CG Training Center has fully air-conditioned and well-lit facilities, including a reception area, administrative offices, and four training classrooms. The Institute has a student lounge with a coffee machine, refrigerator, sink, tables and chairs, and bottled water. Restrooms are available. Parking is provided in a well-lit parking area.

Dispute Resolution Policy

In the event of a dispute—whether on or off the Institute's premises, during or outside Institute hours—that involves issues related to the Institute, the following steps must be taken:

  • Contact person in case of disputes with the school;
  • Timeframe to report issues;
  • Timeline the administration will follow to resolve the issues;
  • Procedures both the student and the administration will follow to resolve the concerns.

Student Responsibilities and Final Agreement

The student agrees to maintain regular attendance and comply with the Institute’s rules and regulations. The student understands that regular attendance is their responsibility and that the Institute’s policy regarding absences and make-up work, as described in the Institute’s catalog, will apply. Violation of the Institute’s rules and regulations may result in dismissal.

Information regarding post-training job placement is available at www.njtopps.com. In the event of an unexpected closure of the Institute, students enrolled at the time of the closure must contact the Training Evaluation Unit of the New Jersey Department of Labor and Workforce Development within ninety (90) calendar days. Failure to do so within that time frame may disqualify the student from any available assistance. Contact the unit at trainingevaluationunit@dol.nj.gov.

The student acknowledges having received a copy of this agreement, the Institute’s catalog, and written confirmation of acceptance prior to signing this contract. By signing this contract, the student acknowledges having read and understood the terms and conditions and agrees to them.

The Institute’s signature on this contract serves as written confirmation that the student named above has been accepted into the above-mentioned program(s). This agreement is not binding until three business days after it has been signed by both parties. Both the student and the Institute will retain a copy of this agreement.
